Opened in 1964 as the only skilled-care facility in Minnesota exclusively for men, Mission Nursing Home provides care for up to 65 residents who experience a variety of physical and behavioral health issues.

SERVICES
Mission Nursing Home is the only all-male skilled nursing facility in Minnesota, located on beautiful Medicine Lake in Plymouth, Minnesota.
We have highly trained licensed staff capable of assessing, managing, and treating all levels of skilled care, along with a wide range of co-morbidities. We excel at wound care including wound vacs, drains and irrigation; respiratory care including established tracheotomies, BiPap and C-Pap management, nebulizer treatments and oxygen therapy; post-acute management of trauma, surgical or critical illness; paracentesis; IV treatments and management including pre-approved IVP medications.
We have a strong therapy department offering physical, occupational and speech therapies for rehabilitation services to those in need.
We provide behavioral health management to all skilled residents in need and have Psychiatry Services on-site with weekly visits from psychology.
Residents are able to participate in the Gazebo program if approved by a physician. This program is a privilege for residents that choose to enjoy an alcoholic beverage from time to time, or daily, pending physician approval. Residents that have physician orders to participate may enjoy a prescribed amount of alcohol during the daily happy hour.
Mission Nursing Home offers an indoor smoking area for cigarettes only. For those who enjoy pipe smoking, vaping or cigars, these can be enjoyed outside.

History
Mission Nursing Home opened in 1964 as the men of Mission Farms, part of Union City Mission (now Missions Inc. Programs) started to age and required long-term skilled nursing care.
The facility was at one point housed in the Union City Mission Chapel before it was torn down in 1995 and patients were moved to new facility.
Today, the facility provides services for up to 65 men who require long-term and short-term skilled nursing care. As the only all-male facilities in the state, our unique nursing facility can provide specialized care for men who also experience behavioral issues.

Paul's Story: A journey to independence
When Paul arrived at Mission Nursing Home he was flat on his back, he couldn’t move. He had a feeding tube, a tracheotomy tube and several very serious bedsores. He was in a lot of pain and very anxious.
In spring 2020, Paul was in a car accident. He found himself being moved from nursing facility to nursing facility and caught COVID-19 twice. In July 2020, he was moved to Mission Nursing Home.
At Mission Nursing Home, Paul's bedsores began to heal and our nursing and physical therapy staff worked with him to gain his independence back. They got him the proper wheelchair and accessories and taught him how to use them. They pushed him on days he felt unmotivated so he wouldn't lose progress.
After more than a year at Mission Nursing Home, Paul has gained much of his independence back. He sits upright in his wheelchair. He can leave the nursing home to go to appointments by himself. Paul says he's in less pain and he is less anxious.
He is now working with staff to transition into a group home for more permanent housing. Staff say he is a far cry from the man they met in July 2020 and his story inspires them.
Mission Nursing Home Staff
Lance has been a Nursing Assistant at Mission Nursing Home since 1995.
He says after 26 years, "it's become a second home."
Mission Nursing Home is unique because of the "communication between staff and residents, the quality of care and the teamwork of staff," says Lance.
